SP License Application Instructions Video - Updated script

Welcome to the Financial Services Commission of Ontario's Service Provider Application instructional video.

All Service Provider's licence applications will be completed online through the FSCO website at www dot fsco dot gov dot on dot ca.

Before you begin, you will need to know how your business is structured in order to complete and submit the correct application type. For this demonstration, we will be showing how to file a sole proprietorship application.

From FSCO's home page, select "Apply Now for a Service Provider Licence." If you have not started the application process, select the appropriate application link under the "Apply Now" section. If you have already started an application, log in to your existing FSCO account to complete the process.
In this case, I will select the "Sole Proprietorship" application link.

You will be directed to the FSCO Account Log In page. The first time you come to this page, you will need to create a FSCO account. This account will provide you with access to FSCO services for licensees and other relevant information. It will also serve as a central hub for your communications with FSCO.

All applicants will require an account in order to submit a licence application. Select the "Create a New FSCO account" link to continue.

The account should be set up in the name of the individual who has been designated the Principal Representative for the Service Provider. The Principal Representative is also responsible for completing the application and attestation.

Moving forward when we reference "You," we are referring to the Principal Representative.

To begin, we will require you to provide your name and a valid email address.

You will need to create a password that is a minimum of eight characters. It must have at least one capital letter, one number and one symbol.

You will now need to provide the answers to 2 security questions that will be used in case you forget your password.

Select the "Proceed" button.

Now that your account has been created you may continue with the Service Provider application by selecting "Continue what I was doing."

Before proceeding, please ensure you have the following information on hand:

If you operate under a trade name, have a copy of the trade name registration as you will be required to enter the name and expiry date as it appears on the related master business licence.

You will also require the HCAI registration number of all facilities that will be part of this licence. As well, you will need to have the addresses of all locations where you perform or provide SABS related goods and services.

As part of the application process, you will also be required to provide the total number of unique SABS claimants that you received a payment for from an insurer based on an OCF-21 submission in the 2013 calendar year.

The fee for licensing is broken down into two parts - a one-time application fee of three hundred thirty seven dollars (337) and a regulatory fee which is based on the following formula:

One hundred twenty eight (128) dollars multiplied by the number of HCAI facilities that are part of the application plus fifteen (15) dollars multiplied by the number of unique SABS claimants that the applicant was paid for by an insurer based on an OCF 21 submission in 2013.

Please proceed by selecting the "Next" button.

If the FSCO account was not created in your legal name it will need to be changed on this page. If it has been created in your legal name then you may proceed by providing your date of birth, gender, email address and phone number where FSCO can contact you for questions with respect to the application.

If you, as the Principal Representative, are a regulated health professional, click "Yes." You will need to provide the name of the Regulatory College your profession is governed by, your registration number and the health discipline you are regulated in. If not, click "Next" to proceed.

During this step, you will be asked to provide information about the business including its mailing address in Ontario and any trade names. Only questions marked with a red asterisk are mandatory.

At any time, if you wish to save this application and return to it later, you can click "Save as draft." The application will be saved and be available in your FSCO account. If you quit the application, you can access the saved draft under your active files by clicking on the file name. You can then continue with the application where you left off.

Once the required information has been completed, you can click "Next" to proceed to the next step.

In this section, you will find information about the applicant's regular screening process for suitability including a criminal record check. Please read these instructions carefully before you complete the next set of questions.

In step five, you will be asked to provide information regarding any previous charges, offenses, complaints and any enforcement actions that may have been taken against your business in any jurisdiction, state or country. If you answer "Yes" to any of these questions, you must provide an explanation in your own words, as the case may be. The information provided will be considered by FSCO when determining applicant's suitability to carry a licence. Click "Next" to proceed to step six.

This step collects details about the Principal Representative and the business, including name of the insurance company whose Preferred Provider Network your business may be a member of. If this is applicable to your business click "Yes," otherwise click "No" to proceed.

In the next step, you will be required to provide further details about your business such as the number of regulated and non-regulated health professionals working for the business and the services provided by them as they relate to the SABS claimants.

You will also be asked to provide information about each facility registered with HCAI such as HCAI registration number, address and any trade names, as applicable. You must have a minimum of one facility enrolled in HCAI if the business registered under the Ontario mailing address shown on the screen does not provide any SABS related goods or services.

Lastly, you will need to provide the total number of unique SABS claimants the business dealt with in the previous calendar year.

Click "Next" to move to step eight.

In this step, you will be required to provide information about the business' records and billing practices such as reconciliation of billing and financial statements. Once completed, click "Next" to proceed to step nine.

This step identifies details about the business standards and practices as it relates to invoicing insurers for listed expenses incurred under the SABS. If you answer no to any of these questions, you will be required to provide an explanation. Once the answers have been provided, click "Next" to proceed to the review stage.

In this step, you will be able to view an electronic PDF version of the application you just completed by clicking "View Application." You are encouraged to review the application to ensure that the answers provided are accurate and complete. If you need to make any changes, you can go back to the application by clicking "Previous."

In step 2 of the Review Stage you will be able to preview fees associated with this application. Fees are calculated based on the number of locations and the number of unique SABS claimants you provided in the application. Regulatory fee is pro-rated to 4 months.

Read the instructions provided in this step carefully. You will be asked to consent to the collection of information and authorize FSCO to use this information, as necessary, to determine if the licence should be granted. You will also attest that the information provided is true and accurate to the best of your knowledge, and as complete as possible from every perspective. You must agree to the consent and attestation to proceed to the payment stage. Click "Yes" to continue.

Please note that once you complete the attestation, you will no longer be able to make changes to your application. You can only view a read-only copy of the application in PDF format.

You will be provided with an itemized view of the licensing fee. When ready to continue with the payment, click "Pay Now Using Debit/Credit Card." A cheque payment is required if the licensing fee is greater than five thousand dollars.

If you are ready to proceed with the payment, select "Pay and Submit."

In this case, you may choose to pay by credit card. Enter the credit card holder's name, the credit card number, expiry date, security code, and click pay.

Once the payment is made, you will receive a confirmation and be able to view and/or print the payment receipt. We recommend that you make an electronic copy of this receipt, for your own records, prior to submitting the application.

Please note that the application will not be submitted if the payment fails. Applications paid by cheque will be considered as submitted pending payment. FSCO will not process these applications until the cheque, including the remittance slip, is received.

To submit the application to FSCO, click "Next." The status of the application will change from "Draft" to "Submitted to FSCO."

Once you submit the application, you will be able to view the answers and monitor its status on the FSCO portal by logging into your existing account. The submitted application can be found under the "Active Files" section. While FSCO is processing the application, its status will be "Under Review." If the licence application is approved, you will receive a notification in your Message Center and the application status will be changed to "Approved."
